We also provide care coordination services for qualifying residents of St. Louis County. Care Coordinators assess individuals to determine needs and find what services, if any, are needed to ensure a full and fair chance to thrive. We can:

  • Help answer questions about available services.
  • Work with providers to help people get the care they need.
  • Help providers share information with each other.
  • Ensure people get home and community-based services they need.

St. Louis County Age-Friendly Survey

Once enrolled in the AARP Age-Friendly Network of States & Communities, the first step is to conduct a community needs assessment. The age-friendly community survey gives residents the chance to highlight what matters most to them and voice what changes they want to see.


Emergency Preparedness

Emergency preparedness is essential for the health and safety of all residents, especially older adults who may face unique challenges during emergencies. With the support of the Age-Friendly Minnesota grant, we are working to bring emergency response to the forefront of our aging populations and ensure that older adults are meaningfully integrated into all aspects of emergency planning, response, and recovery.

Mental Health 

Loss is more than the death of a loved one. Ambiguous losses are often not recognized or acknowledged. Grief from these losses can be difficult to navigate and find support for. Stigma plays a role in this, as grieving a death loss has always been more socially accepted and understood.
