Minnesota Senior Health Options (MSHO) and Minnesota Senior Care plus (MSC+) program
St. Louis County Public Health professionals provide care coordination services for the residents of St. Louis County over the age of 65 with Medical Assistance eligibility and enrolled with a managed care organization. Care Coordinators assess individuals to determine their needs and what services are necessary to keep them in the least restrictive environment, whether it’s at home, foster care, assisted living or skilled nursing facility. Care Coordinators help answer questions, work with doctors to help people get the care they need, help doctors and other providers share information with each other and assure people get the home and community-based services they need.
