The American Diabetes Association has designated St. Louis County as a Health Champion for our work to prevent diabetes!
Welcome to Total Wellness Total Transformation! This program works by you taking the first step and registering. There is no cost for this program. When you enroll in the program you will get all the materials you need which includes your program guide, tracking card as well as a Total Wellness logo item.
Your Wellness program now offers you many ways to become involved. There are five categories of wellness so that everyone feels they can participate in some way.
1. "I Choose Prevention" is geared towards encouraging you to take preventative steps in keeping yourself healthy, such as physical exams and screenings.
2. “I Choose a Healthy Lifestyle” is taking time for you to learn about your health and to encourage you to have an active and safe lifestyle.
3. “I Choose a Healthy Community” helps you find ways to help in your community. The simple act of helping has been shown to make people feel happier with their lives.
4. “I Choose Financial Wellness” helps you find ways to manage your finances and have a healthy bottom line.
5. “I Choose Healthy Programs” provides you with opportunities to participate in some fun and healthy programs.
Who Can Participate?
Note: If you or your family member is not a member of the St. Louis County Self-Insured Health Plan and are not required to be, you will need to substitute your own insurance company's health assessment for the Blue Cross Blue Shield online health assessment referred to on the tracking card.

Weight Loss Reimbursement Program
County/ARC employees and/or their spouses
(or one other adult in same household)
are eligible for up to a $250 ($500 per household)
All you need to do is:
Contact Tiffany Kari, Health Promotions Coordinator, at 218-720-1551 for a listing of approved programs.
Complete at least six weeks of the program.
Fill out an Employee Voucher.
Voucher tips: Complete the claimant's portion and be sure to sign and date it. Also indicate your department and location by your name. Attach your supporting documentation (receipts) and return to Employee Development, GSC, 2nd floor attn: Julie Geissler for approval. Note: Participants also qualify for points on their wellness tracking cards when they join one of these programs. Reimbursements are available to participants who are enrolled in the wellness program as well as those who are not.
BluePrint for Health® Fitness Discount
All members of the St. Louis County Health plan administered by Blue Cross and Blue Shield of Minnesota are eligible for the BluePrint for Health® fitness discount. That means that you and one other eligible adult in your household, if you have a family plan, can get up to $20 credit toward your monthly fitness center fee when you work out at least eight days per calendar month. That’s up to $40 in credits a month.
All you have to do is show your ID card with the fitness icon at a participating fitness center and ask to be enrolled.

You can search for a convenient club using your zip code by clicking here. You may also contact your fitness center to find out if they participate.
Work out eight days per month
Your visits are automatically tracked. As long as you remain eligible and meet the eight-day per month workout requirement, you’ll receive a monthly credit or reimbursement through your fitness club or local banking institution.
First credit/reimbursement can take 60 days
There is a period of time between the month of the completed workout activity and the month you get credit. For example, if you work out eight times in October, it will be verified in November, and credit/reimbursement will occur in December.
Work closely with your fitness center
When you enroll for the program at a participating fitness center, make sure the fitness center has correct information and your activity is tracked each month. Also, you can confirm with your fitness center what their credit schedule is for their members.

Small Group Personal Fitness Challenge
St. Louis County’s Health Insurance Committee is excited to share that efforts behind the Small Group Personal Fitness Challenge have paid off! Data obtained through 230 participant’s pre- and post- fitness evaluations shows that participating in small group fitness sessions led by a certified professional trainer can result in many health benefits including increased flexibility, weight loss, motivation and self-confidence. The outcomes of this challenge are highlighted in a documentary. In recognizing the likely benefits to the self-insured health plan, the insurance committee agreed by consensus to add a new small group personal fitness reimbursement of $50 each month.
All you have to do to join/benefit from this challenge is to gather together a group of eligible individuals and contact the facility of your choice.
