Rates are unique to each enrollee.  They can be based upon and vary by hours worked,  wage rate and age.  The Enrollment Form contains a worksheet you can fill to estimate your premium.  If your labor contract provides for an employer paid contribution to your LTD premium, use the 30%/70% worksheet.  Otherwise, use the 100% employee paid worksheet.
