Special Open Enrollment for 2012

In a continuing effort to minimize ineligible claims expenses against its self-insured health and dental plans, St. Louis County is conducting a mandatory insurance “positive re-enrollment” for coverage effective January 1, 2012.  To continue existing coverage in most medical and/or dental plan(s), all enrolled employees and retirees are required to complete and submit a new Group Coverage Form.  It’s understandable that completing a new enrollment form seems unnecessary, but it’s a simple step that can significantly improve the sustainability of our health and dental plans.

The Group Coverage Form should be completed and sent to:

If you submit a scanned signed copy of your form via this email address, please retain the original for your records, do not submit the physical form. 

Please do not contact us to inquire if your form has been received.  You will be contacted if your form was not received.

RE-enrollment Tips RE-enrollment FAQs

Tip#1:  This is a re-enrollment so you must re-enroll yourself and your dependents so you will need to "ADD" all coverages that you want to continue or keep.  

Tip#2: Be sure to check the "Open or Special Enrollment" box under the "Check ALL that apply" Section of the form.

Tip#3: If you are continuing or keeping your same dependents, you still need to list them on page 2 and "Add" them.
