The goal of Elderly Services programs in St. Louis County Public Health and Human Services is to assess the needs of our individual elders, to provide appropriate home services to enable them to remain at home for as long as possible, and to keep them as independent as possible in the least restrictive placement when the home is no longer a safe alternative. Programs designed for Seniors are:
Long Term Care Consultation (LTCC):
Public Health Nurses and Social Workers assess the appropriatness of nursing home placement and provide information and resources for alternative living arrangements or in-home services. This is a free service offered to help determine the kinds of limitations a person has and the services available to help meet their needs.
Elderly Waiver Program (EW):
The Elderly Waiver (EW) program funds home and community-based services for people age 65 and older who are eligible for Medical Assistance (MA) and require the level of medical care provided in a nursing home, but choose to reside in the community. The Minnesota Department of Human Services (DHS) operates the EW program under a federal waiver to Minnesota's Medicaid State Plan. Counties administer the program.
What types of services are available?
Covered services include visits by a skilled nurse, home health aide, homemaker, companion, personal care assistant, as well as home-delivered meals, adult day care, supplies and equipment, home modifications, and certified community residential services (assisted living, foster care, residential care). Alternative Care program (AC):
The Alternative Care (AC) Program is a state-funded cost-sharing program that supports certain home and community-based services for eligible Minnesotans, age 65 and over. This program provides home and community-based services to prevent and delay transitions to nursing facility level of care. This is a program for those who have either too much income or assets to qualify for Medical Assistance. Depending on income, a small “Premium” is charged each month for coverage.
